Working alongside the clinical team

We do not work in isolation. Palliative packages are coordinated with district nurses, the GP, hospice-at-home teams and Macmillan or Marie Curie services where they are involved, so that everyone is following one plan.

What matters at this stage

Pain and symptom comfort, dignity, mouth and skin care, gentle repositioning, and presence — for the person and for the family sitting with them. We also support the practical side that families rarely have capacity for.

How a package tends to change

Palliative care rarely stays static — a package that starts as a daily visit often moves to multiple visits, then to waking night cover, as needs progress. We plan for that trajectory with the clinical team from the start rather than reassessing from scratch each time, so the increase in support does not lag behind the increase in need.

How we train for it

Carers working in palliative and end-of-life care are trained in comfort-focused personal care, recognising and reporting changes that the clinical team needs to know about, and supporting a family that is grieving in real time — including the practical and emotional difference between supporting someone and supporting the people around them.

Palliative & end-of-life care: what people ask

How quickly can palliative care start?

As quickly as the situation needs — this is one of the few areas where we prioritise getting support in place fast over a lengthy assessment process, while still coordinating properly with the clinical team already involved.

Who is actually funding this — us, the NHS, or the family?

It depends on the situation. Some palliative care is funded through NHS Continuing Healthcare, some through the council route, and some privately. See paying for care, or ask us directly — we would rather have that conversation early than let cost become an added worry at an already difficult time.

Do you work with our hospice or Macmillan nurse?

Yes — palliative packages are coordinated with whoever is already involved: district nurses, GP, hospice-at-home teams, Macmillan or Marie Curie, so everyone is following one plan rather than working around each other.

What support is there for the family, not just the person being cared for?

Respite so family members can rest, someone to sit with during a difficult night, and practical help with the parts of caring that are exhausting to do alone. Ask what would actually help — the answer is rarely the same for two families.
